The Health Center Manager prepares center budgets, places controls over budget expenditures, etc. Is responsible for the overall management of the site’s human resources by ensuring that staff is well trained, competencies are checked, and evaluated regularly. This position represents the organization for the purpose of dealing with licensing requirements and the requirements of regulatory agencies. Measures and evaluates programs and processes with the health center along with regularly participating in UHC’s Quality Improvement Program. Maintains effective communication with the business unites of the organization, external community groups and agencies, and state and federal agencies. Is expected to be and active and visible member of the community in which their site is located and to be that area’s primary representative for United Health Centers.

This position is responsible for designing, coding and modifying web pages from layout to function according to department and corporate specifications. Creates visibly appealing sites and features that have a user-friendly design and features for clear navigation that leverages the visual impact of the overall corporate identity.   The Web Programmer participates in numerous in-house graphic projects and corporate events to understand and compliment social media and print media development. The Web Programmer is responsible for collaborating and creating a variety of products including links to other source websites, utilization of photos, videos and print graphics as needed.

The Associate Social Worker (ASW) will provide UHC patients with case management and access to services. The ASW is responsible for the prevention and early intervention services of individuals presenting in early manifestation of mental illness. The ASW will work in collaboration with the primary care medical team in coordinating all psychosocial community services under the general direction of UHC's Behavioral Health Director.   SUMMARY OF RESPONSIBILITIES: Works closely with the primary care provider and other members of the patient care team to: - Provide direct care to UHC patients with behavioral health problems or social service needs. - Assist primary care providers in consultative contact for patients by assessment of patient needs through "warm handoffs". - Collaborate with the UHC perinatal services program for referral and treatment of patients in need of psycho-social and social service care. - Assists with program development through creation and revision of departmental policies, procedures, and protocols. - Demonstrates exemplary customer service in both verbal and written encounters. - Maintains direct communication with Health Center Managers, providing updates on operational issues, process improvement suggestions and other concerns. - Under the direction of the QA Coordinator, will participate in process improvement and data collection as necessary. - Provides coverage in various health centers as necessary. - Interfaces with other departments to ensure operational effectiveness. - Able to multitask and manage assigned projects as directed. - Other duties and responsibilities as assigned.

Primary tasks are to assist the Health Center Manager at the site. LVN's may be required to draw and collect blood samples from patients and prepare specimens for laboratory analysis. Finally, the LVN position provides or facilitates the provision of information needed by both patients and clinicians. Furthermore, they ensure a smooth and well-connected patient flow. The LVN reports to the Health Center Manager.
