Overview

We are hiring for a Full Time, Outreach Specialist for our growing team. 

 

The position reports to the PACE Program Director. The primary responsibility of the PACE Outreach Specialist is the ongoing recruitment of PACE participants into the program and coordination with the interdisciplinary team to facilitate enrollment. In addition, this position is responsible for all community awareness efforts of the program, including the marketing of services to providers, referral sources, and the community at large to encourage knowledge of the PACE program and growth and maintenance of an optimal enrollee census.

Responsibilities

PERFORMANCE AREA 1:

  • Develops and implements annual marketing plan to achieve enrollment goals.
  • Plans and coordinates advertising and promotional campaigns and strategies for the PACE Program. 
  • Answers calls from prospective participants and explains the UHC PACE enrollment process.
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties
  • Prioritizes customer service and support for the participant population served, their caregivers and the interdisciplinary team.
  • Schedules intake appointments for enrollment team and, when needed, conduct intake appointments.
  • Conduct tours of the program and serve as a host for prospective participants and families
  • In collaboration with UHC Creative Department, coordinates the production of printed publications and audio-visual productions, including developing design, layout, and text for informational materials such as newsletters, bulletins, pamphlets, directories, and audio-visual productions. 
  • Initiates, develops, and maintains public contacts for disseminating program information. Drafts news releases that are submitted for higher-level approval through UHC’s Creative department
  • Establishes rapport and maintains relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
  • Provides education referral sources and educates the community on PACE services.
  • Communicates collaboratively with the interdisciplinary team with updates about potential enrollees, opportunities for existing program development and other feedback on related efforts.
  • Participates in quality improvement.
  • Participates in public relations to support goals including television, radio, press, and public speaking opportunities,
  • Maintains administration and relevant reporting, tracking, and planning systems.
  • Plans and coordinates all marketing resources within an approved budget.
  • Responsible for supporting the health care team through community outreach, enrollment assistance, and education activities, as assigned. 
  • Provides information on health insurance, safety programs, and community resources.
  • Attends relevant community meetings as assigned. 
  • Organizes and participates in network-hosted events, community events, and presentations.
  • Provides training on health-related topics as assigned.
  • Provides health screenings at events.
  • Responsible, as assigned, to support the implementation of PACE programs.
  • Supports the implementation of program curriculum and/or objectives in community settings.
  • Supports supervisor in the collection of data and reporting.
  • Performs other duties as assigned.

 

PERFORMANCE AREA 2: General Corporate Expectations

  • Attends and actively participates in all meetings (e.g., department meetings, program meetings, employee staff meetings) and other activities as required or assigned.
  • Attends workshops/seminars as necessary to increase skills and knowledge to provide effective care, treatment, and/or leadership.
  • Supports the overall needs of the PACE center by working flexible or extended hours when necessary.
  • Supports the needs of the health center by traveling to other UHC clinics when staffing needs dictate the need for personnel.
  • Demonstrates awareness of, and compliance with, organizational mission and objective of UHC to provide health care access and support services for all members of the community.
  • Supports their own staff development by completing the required hours of continuing education each year.
  • Other work-related duties as assigned by supervisor. Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.
  • Maintains confidentiality and respect for information regarding patients and other team members; abides by UHC Rules of Confidentiality and general HIPAA regulations regarding privacy.
  • Displays a positive, professional and respectful demeanor at all times toward employees, peers, professional contacts, and participants served, maintaining a professional appearance and positive image for the health centers.
  • Contributes to the team by promoting positive staff interaction, maintains open communication throughout PACE center staff.

Qualifications

QUALIFICATION REQUIREMENTS:

EDUCATION:

 

  • Bachelor’s Degree in Business or related field required;

PRIOR EXPERIENCE / SKILLS:

  • Have a minimum of two years of experience in community outreach, marketing, educational activities, and customer service (preferred).             

CERTIFICATIONS / LICENSES:

  • Current BLS CPR        
  • Possess a valid Driver's License                                                                                            

SKILLS:

  • Bilingual preferred but not required.
  • Able to quickly build and maintain rapport with participants and providers of differing backgrounds; team player.
  • Customer-service oriented
  • Strong computer skills
  • Attention to detail and excellent follow-through on work tasks
  • Able to handle multiple tasks simultaneously
  • Highly reliable and timely.
  • Must perform duties as assigned.

 

The hourly wage range that UHC reasonably expects to pay for the position upon hire starts at $32.31 an hour. Our salaries are dependent on knowledge, skills, and experience.
