We are hiring for a Full Time, Intake and Enrollment Specialist for PACE.

The PACE Intake and Enrollment Specialist serves as a key point of contact for prospective participants, families, caregivers, referral sources, and community partners. This role guides individuals through the PACE intake and enrollment process by providing education about the PACE model, explaining eligibility requirements, conducting initial screenings, coordinating assessments, supporting completion of enrollment documentation, and facilitating timely handoffs to clinical, eligibility, outreach, and interdisciplinary team members.

The position requires strong knowledge of healthcare or senior services, experience working with frail or geriatric populations, and experience working with frail elderly in the healthcare profession. The Intake and Enrollment Specialist must be able to communicate clearly and compassionately, maintain accurate and timely documentation, manage multiple priorities, and support prospective participants and caregivers through a process that can be complex and highly personal.

In addition to intake and enrollment coordination, the role supports approved outreach and community engagement activities, tracks enrollment activity and trends, participates in team meetings and events, and helps ensure compliance with CMS, state-specific PACE requirements, HIPAA, privacy standards, and organizational policies. The position works collaboratively across marketing, outreach, clinical, administrative, and leadership teams to support enrollment goals while maintaining a participant-centered, culturally competent, and compliant approach.

PERFORMANCE AREA 1: Works closely with the Interdisciplinary Team and leadership to: 

• Conduct initial screenings and assessments to determine program eligibility.
• Utilize proven expertise to support the assessment process.
• Assist with the completion and submission of enrollment applications and required documentation.
• Schedule and facilitate home visits, center tours, and interdisciplinary team evaluations.
• Maintain accurate and timely records in compliance with HIPAA and regulatory standards.
• Collaborate with marketing, outreach, and clinical teams to support enrollment goals.
• Track and report enrollment metrics and trends to leadership.
• Provide culturally competent and compassionate support to diverse populations.
• Ensure compliance with CMS and state-specific PACE enrollment guidelines.
• Provide administrative support as needed.
• Collaborate on outreach initiatives to promote the center’s resources within the community.
• Serve on various internal and external committees as required or designated.
• Participate in team meetings, caregiver groups, and special events as needed.
• Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
• Meet department and company performance and attendance expectations.
• Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
• Ability to partner with the larger PACE team in supporting Reception, Transportation, Dietary, Scheduling and Patient Care Assistants as needed.
• Perform other duties as assigned.

• Attends and actively participates in all meetings (e.g., department meetings, program meetings, employee staff meetings) and other activities as required or assigned.
• Attends workshops/seminars as necessary to increase skills and knowledge to provide effective care, treatment, and/or leadership.
• Supports the overall needs of the PACE center by working flexible or extended hours when necessary.
• Supports the needs of the health center by traveling to other UHC clinics when staffing needs dictate the need for personnel.
• Demonstrates awareness of, and compliance with, organizational mission and objective of UHC to provide health care access and support services for all members of the community.
• Supports their own staff development by completing the required hours of continuing education each year.

• Maintains confidentiality and respect for information regarding patients and other team members; abides by UHC Rules of Confidentiality and general HIPAA regulations regarding privacy.

• Contributes to the team by promoting positive staff interaction, maintains open communication throughout PACE center staff.
• Works flexible or extended hours where necessary.

QUALIFICATION REQUIREMENTS:

EDUCATION/LICENSE/CERTIFICATION:

• CPR (BLS or ACLS) required.
• Valid California Driver’s License is required.

PRIOR EXPERIENCE

• A minimum of 2 years of experience in healthcare, senior services, or enrollment coordination is required.
• 1 year of experience working with geriatric or frail populations is required.

SKILLS

• Bilingual preferred but not required.
• Able to quickly build and maintain rapport with participants and providers of differing backgrounds; team player
• Strong attention to detail, ensuring accurate enrollment coordination.
• Excellent organizational skills and the ability to manage multiple tasks effectively.
• Ability to work collaboratively within a team environment.
• Proficiency in using various software applications, including Word, Excel, PowerPoint, and Outlook.
• Capability to work independently with minimal supervision

The hourly wage range that UHC reasonably expects to pay for the position upon hire starts at $29.28 an hour. Our salaries are dependent on knowledge, skills, and experience.
